Quick answer
GLP-1 injections are given under the skin of the abdomen, thigh or upper arm, usually once a week. Use a new needle each time, rotate sites, inject at the same time each week, and store unused pens in the refrigerator. Always follow the instructions for your specific pen and ask your nurse or doctor to demonstrate the first injection.
Most GLP-1 medicines for diabetes and weight management are given as an injection under the skin. The needle is tiny, and the technique is easy to learn. Doing it correctly helps the medicine work well and keeps you comfortable.
How often are they given?
-
Weekly injections: Semaglutide and tirzepatide for diabetes and weight are given once a week, on the same day.
-
Daily injections: Some GLP-1 medicines are taken daily.
-
Tablets: Oral semaglutide is taken once daily on an empty stomach.
Your doctor will tell you which schedule applies.
Where can you inject?
-
Abdomen: At least 5 cm away from the navel. It is a common, easy site.
-
Front or outer thigh: Good for self-injection.
-
Back of the upper arm: Easier if someone else gives the injection.
Rotate within the same area, moving at least 2 to 3 cm from the previous spot. Avoid scars, bruises, moles, lumpy areas or places where the skin is hard.
What are the steps?
-
Wash your hands. Gather the pen, a new needle and an alcohol swab.
-
Check the pen. Read the label, the expiry date and the colour and clarity of the medicine.
-
Attach a new needle. Remove the paper tab, screw or click it on, and remove both caps.
-
Prime if needed. Some pens need a small test dose. Follow your pen's instructions.
-
Dial the dose. Check the dose window to be sure it is correct.
-
Clean the skin. Let it dry.
-
Insert the needle. Insert straight in. You can gently pinch the skin if you have little fat.
-
Press the button. Hold it down and count slowly as instructed, often up to 6 to 10.
-
Remove the needle. Do not rub hard. A small drop of blood is normal.
-
Dispose of the needle safely. Use a sharps container or a hard plastic bottle. Replace the pen cap.
How should you store GLP-1 pens?
-
Store unopened pens in the refrigerator, away from the freezer section.
-
Never use a pen that has frozen.
-
In-use pens may be kept at room temperature or in the fridge for a limited number of days, as stated on your pen's label.
-
Keep away from sunlight, heat and car dashboards.
-
Carry pens in an insulated pouch when travelling, in hand luggage.
-
Do not share pens.
What if you miss a dose?
For weekly injections, follow the label. Many instructions say to take a missed dose if it is within a few days of the due date, otherwise skip it and take the next dose on the usual day. Do not take two doses close together. When unsure, call your doctor.
What common problems occur?
-
Bruising or small bleed: Normal, press lightly with cotton.
-
Redness or itching: Often mild. Rotate sites.
-
Lumps under the skin: From repeated injections in one place. Change sites.
-
Leakage: Hold the pen in place for the full count.
-
Pain: Use a new needle, let the pen reach room temperature before injecting, if the label allowsd and relax the muscles.
When should you see a doctor?
See a doctor if you have swelling, spreading redness, pus, fever, a hard painful lump, a pen that is not working or if you are unsure about your dose. Seek urgent care for allergic reactions like swelling of the face or trouble breathing.
